Before you head to Park Slope Eye, use this checklist to gather your belongings and make a few notes. You can keep it on your phone or use a paper copy.
Check whether you need previous exam records
For a normal first comprehensive exam at Park Slope Eye, you do not need prior eye records. The practice may request them later if necessary.
For a contact lens fitting based on a comprehensive exam elsewhere, bring the complete record of an exam performed within the previous three months. A glasses prescription alone is not enough.
If you are unsure which applies, ask Park Slope Eye before gathering records.
